Consent by Parent/Guardian Policy:
It is Wangaratta Hockey Club Inc. (WHCI) policy that participants under the age of eighteen years (or otherwise legally defined as minors) are required to have written consent from a parent or legal guardian prior to commencing in any association activity. This consent must be given annually.
Except to the extent that the Trade Practices Act 1974 or other legislation applies and cannot by contract be excluded, I agree that it is a term of my child’s participation that the WHCI is absolved and indemnified from all liability however arising from injury or damage, however caused arising out of my child’s participation with the WHCI or in any way due to any negligent act, breach of duty, default and/or omission on the part of the WHCI.
I release and forever discharge the WHCI from all claims (except those claims made and accepted under a relevant WHCI insurance policy) that I may have or may have had on behalf of my child but for this release arising from or in connection with my child’s participation in the WHCI Competition.
I have read, understood, acknowledge and agree to the Junior Players Consent by Parent and/or Guardian Policy including the release and indemnity.
